Encefalitis inmunomediada pediátrica con estatus epiléptico recurrente: reporte de caso

Las encefalitis inmunomediadas en la población pediátrica constituyen un grupo heterogéneo de enfermedades caracterizadas por inflamación del sistema nervioso central mediada por mecanismos autoinmunes. Su presentación clínica puede incluir crisis convulsivas, alteración del estado de conciencia y déficits neurológicos focales, siendo el diagnóstico y tratamiento oportuno fundamentales para mejorar el pronóstico. Caso clínico: Se presenta el caso de paciente femenina de 7 años en el Hospital de Niños Roberto Gilbert Elizalde de Guayaquil, Ecuador, con antecedentes de encefalopatía aguda en estudio, quien ingresa por estatus epiléptico caracterizado por desconexión del medio, desviación de la mirada, movimientos tónico-clónicos y compromiso respiratorio. Presenta historia de hospitalizaciones previas, manejo con inmunoglobulina intravenosa, corticoides y plasmaféresis. Durante el ingreso actual se documenta recurrencia de crisis convulsivas sin evidencia infecciosa, con estudios de laboratorio dentro de parámetros normales y niveles terapéuticos de anticonvulsivantes. Neurología plantea diagnóstico de encefalitis inmunomediada con recaída, indicando terapia de segunda línea con rituximab y estudios de anticuerpos neuronales. Conclusión: Este caso resalta la importancia del reconocimiento temprano de encefalitis inmunomediadas en pediatría, especialmente en pacientes con evolución tórpida y recaídas, donde la inmunoterapia de segunda línea puede ser determinante.
